An allergist is a medical doctor specially trained to diagnose, treat and prevent allergies.
A custom medication is a combination of carefully chosen active ingredients mixed together in certain strengths, resulting in a unique formulation that is tailored to meet the needs of the individual person it was prescribed for.
You must be at least 18 years of age to receive medical treatment and prescription services at this time.
However, we recognize that many children and adolescents also suffer from allergies and we plan to expand our services to meet their needs.
If you are under 18 years of age (or care for someone who is) and would like to be notified when treatment for this age group becomes available, please send us an email at info@allermi.com.
3 weeks after your prescription is sent your way, we will check in to see how you are doing. If you need a formula change, are experiencing persistent symptoms or side-effects, you may fill out a follow-up visit questionnaire. Your Allermi doctor will review it and respond accordingly, with recommendations and potentially a formula adjustment.
You can request a follow-up visit at any time, and we will reach out every 3 months to check in therafter.
We currently address nasal allergies (i.e., allergic rhinitis or hay fever). We are not currently treating any other allergic conditions at this time.
You will receive a prescription for your custom medication only if deemed appropriate and safe by our allergists after they evaluate your intake questionnaire. If there are specific medications that worked well (or didn’t) in the past, please indicate this on your intake questionnaire, so that this information may be taken into account when our allergists make a treatment decision.
We are proud to partner with Safeway Compounding Pharmacy, a state-of-the-art, fully licensed compounding facility located in San Jose, CA. Safeway Compounding Pharmacy has earned the Pharmacy Compounding Accreditation Board’s (PCAB®) Seal of Accreditation, demonstrating that they meet the highest standards of quality and safety. You can contact Safeway Compounding Pharmacy, located at 6100 Hellyer Ave., Ste. 100, San Jose, CA 95138, at 408.227.1098 or at compounding@safeway.com.
You may fulfill your prescription at any compounding pharmacy of your choice. However, our monthly price includes your medication and we are unable to reimburse you if you choose to fill your prescription elsewhere, where the cost will likely be 2-3 times our prices.
We are not able to fulfill prescriptions written for you in the past by another doctor external to Allermi. However, after reviewing your information, our allergists can determine if that medication is still appropriate, and can possibly incorporate it into your care plan.
You can reach out to your allergist at any time with questions about your medication or treatment plan by sending a message to medicalteam@allermi.com. There is no limit on messages to our medical team.

Nope! Allergy testing is not required before signing up for Allermi. We treat the symptoms of allergy, so whether you're allergic to cats, dogs, dust, pollen, grass or more, we can help relieve your symptoms.
Our allergists may suggest formal allergy testing at an allergist local to you, depending on how you respond to treatment.
Even if you’ve had negative allergy testing, Allermi can certainly still benefit you if you're having nasal symptoms. Because Allermi treats inflammation and congestion in allergic and non-allergic nasal passages, even people without identifiable allergies can benefit from the nasal medications in our custom sprays.
Initial improvement in nasal symptoms in response to Super Spray should be prompt (within 10-15 minutes), and should improve even further with regular use over the course of several days to weeks. Peak nasal response might take up to 3-4 weeks.
It is important that during your allergy season, even if you are starting to feel better, you maintain regular use of your Super Spray to help prevent inflammation and congestion from returning.
The length of the course of treatment is dependent on the response of each individual patient and what symptoms are being treated. For example, people with seasonal allergies to pollen might only need to use their sprays for 3 months at a time, while people with chronic nasal allergies might need to use their sprays daily, year-round.
Your nasal spray is designed to last you for one month of treatment with 1 spray per nostril, twice daily. Please note that you should avoid using Super Spray past its expiration date printed on the label.
Your Super Spray formula may contain a very small dosage of decongestant medication. These medications, when sold full-strength in drugstores, often advise 3-day use limits on the label. Because our dosing of decongestant medication is a tiny fraction of that sold over-the-counter and because we combine it with anti-inflammatory and/or antihistamine active ingredients, our Super Spray does not require a 3-day limit and can be used daily.
More than likely, you should be able to continue most existing oral medications. Our allergists will review your medications and notify you if any changes should be made.
While there are no significant interactions between the components of Super Spray and other medications, it is important that you tell our allergists on the intake questionnaire exactly what medications you are currently taking.
Yes. Many of the tubes and vessels of the eyes are connected to the nose, meaning that treating inflammation in the nasal passages with a nasal spray will also relieve inflammation in the eyes, helping to reduce itch, tearing and redness in the eyes.
If you do not experience improvement in your eye symptoms with regular use of the nasal spray, please let your allergist know and they may recommend an over-the-counter eye drop.
Our nasal sprays can help treat the nasal symptoms of COVID-19, such as congestion and runny nose. Please order a new nasal spray once your infection has passed, so as to avoid re-infection.
The Super Spray components can help decongest the nasal passages and reduce the inflammation caused by the immune response to infection - so, it will help treat your stuffy nose during a cold, too!
It is recommended that after a cold or sinus infection, you request a new nasal spray so as to avoid re-infection.
Powdered drugs can be caustic and inflammatory to the nasal tissue and can cause permanent blood-vessel fragility, perforations, erosions and other structural defects or deformities in the nasal passages, that could worsen when exposed to medicated nasal spray. Because the risk of side effects is considerably higher in this setting, an in-person examination by an ENT specialist is important before starting any nasal spray treatment. Because Allermi's medical evaluations are conducted online and we cannot examine the nasal passages in-person with a nasal scope, we do not offer our services to patients with a history of having used nasally-inhaled substances.
When ou see oxymetazoline in your formula, don't be alarmed! Oxymetazoline is a highly effective decongestant medication found at a high dosage in Afrin. Afrin has a 3-day limit on use and are meant to be used very short-term (3 days) for colds or flu, otherwise there is a risk of rebound congestion, i.e., congestion that worsens immediately or shortly after using. At Allermi, we use a significantly lower dosage (less than 1/10th!) of oxymetazoline than Afrin. At the low dosage we use, the risk of rebound congestion is significantly reduced. In addition, when oxymetazoline is combined with anti-inflammatory medication in Super Spray, the risk of rebound is even further reduced, making it safe for daily use.You should not experience rebound congestion with Super Spray and your congestion relief should last 12-24 hours. Please note that it can take several weeks for Super Spray to reach full efficacy, especially if you've taken a lot of Afrin.
Yes. Our three decades of experience in prescribing our multi-ingredient nasal-spray treatment testifies to its sustained effectiveness and its absence of significant adverse reactions with long-term daily use.
Longstanding or repetitive exposure to environmental allergens creates chronic allergic inflammation - most commonly of the membranes of the nasal passages, eyes and throat - that in turn produces the classic chronic symptoms of allergy: congestion, sniffling, runny nose, post-nasal drip, throat-clearing, cough, sneeze and itchiness.
Thus, chronic allergy sufferers usually require medication daily or twice-daily to control their symptoms, so long as they are continually exposed to their allergens.
For pet allergies: use an air purifier with a HEPA filter, vacuum frequently (vacuum should also utilize a HEPA filter), bathe pets at least once weekly, do not allow pets in your bedroom, brush your pet regularly outdoors and keep pets off of clean clothes.
For dust mite allergies: use an air purifier with a HEPA filter, vacuum frequently (vacuum should also utilize a HEPA filter), use allergen-proof mattress, pillow covers and bedding, and keep decorative pillows and stuffed animals off the bed and out of the bedroom.
Super Spray is our custom prescription nasal spray. It contains multiple active ingredients (at the right percentages for you) that each treat a different component of nasal allergy symptoms.
We use custom combinations and doses of up to 4 of the following active ingredients:
-Oxymetazoline (decongestant) for congestion and stuffy nose (note: we use micro-doses of this ingredient to prevent side-effects)
-Azelastine (antihistamine) for sneeze and itch
-Ipratropium (anticholinergic) for runny nose and post-nasal drip
-Triamcinolone (anti-inflammatory) for allergic inflammation
Salinity Spray is our all-natural nasal saline mist that hydrates, soothes and cleanses the nasal passages.
Allermi does not dispense oral antihistamines or any other oral medications. However, oral antihistamines can be helpful medications in reducing nasal and eye itch, as well as sneeze.
Your allergist may recommend that you take an oral antihistamine pill for a short period of time in addition to your nasal sprays, as you adjust to the new treatment, depending on the nature of your symptoms. Oral antihistamines are available online and at most pharmacies.
Allermi does not dispense oral decongestants or any other oral medications at this time. Oral decongestants, such as pseudoephedrine or phenylephrine, are the active ingredients in medications like Sudafed, Mucinex-D, Allegra-D, Claritin-D and Zyrtec-D.
Oral decongestants can cause significant cardiovascular side effects, as well as excessive dryness of the mucous membranes, irritability and insomnia. Because of the high risk potential, our allergists do not recommend taking oral decongestants.
Yes! Because many people experience exacerbated nasal symptoms during air travel, using the nasal sprays before takeoff and descent of the aircraft can help prevent the discomfort caused by cabin-pressure changes felt in the head, face or ears. Plus, all our products meet the volume regulations for air travel, so you can keep them with you onboard.
Please make sure you are regularly following up with your doctor about your asthma to ensure it is properly controlled. While treating upper airway/nasal symptoms can potentially indirectly help with lower airway/asthma symptoms, Allermi is not an asthma treatment.
Please be sure to follow up with your ENT physician regarding your nasal polyps. Nasal polyps have the capability to reduce the effectiveness of Super Spray, since polyps might block the ability of the spray to reach the high and deep nasal lining. While Super Spray can help reduce the size of polyps and reduce overall nasal inflammation and reactivity, you might need additional intervention by an ENT specialist if your symptoms do not improve with consistent Super Spray use.
Please let us know immediately if you experience any change in urinary symptoms after starting the Super Spray, such as: difficulty starting to urinate, urinating more frequently, incomplete voiding, or painful urination.
While it's rare, one of the ingredients in many Allermi formulas, azelastine may increase drowsiness when taken with sedating medication. If you experience an inconvenient or bothersome increase in drowsiness, please let us know.
There are several steps you can take to help lessen the symptoms of GERD/reflux, which is often associated with post-nasal drip:
1) 6"" elevation of the head of your bed frame using solid blocks (available online as bed ""risers""). Alternatively, sleeping on a wedge pillow. This should not be underestimated!
2) Avoid irritating food (acidic foods such as citrus, vinegars, many spices, peppers, caffeine, alcohol)
3) Avoid eating in the 3 hours before bedtime.
4) Medications such as Pepcid/famotidine (milder acid reduction) or a PPI (proton-pump inhibitor), such as Prilosec, Prevacid or Dexilant.
5) Diagnostic evaluation by gastroenterologist.
While some Super Spray formulas contain oxymetazoline (the active ingredient in Afrin) to treat congestion, it is always at a micro-dose - a significantly lower concentration than OTC Afrin sold in stores. Therefore, the risk of rebound congestion and other side effects is significantly reduced and Super Spray can be used up to twice daily for as long as you need.
It has been studied and shown that, at this tiny a dosage, oxymetazoline should not cause rebound congestion, even with twice daily use long-term. In addition, the micro-dose of oxymetazoline is combined with anti-inflammatory ingredients in the same bottle, which has also been studied and shown to drastically reduce the risk of rebound congestion.
Please see our Science section for more information and to read the associated studies.
You can safely continue taking montelukast/Singular for your nasal allergy symptoms while using Allermi. It is possible that you may find you no longer need montelukast/Singular for nasal symptoms after you have established regular treatment of your Allermi.
Although your allergies may indeed be contributing to your cough, when coughs are prolonged it is possible there is more than one cause. If your cough does not improve along with your other allergy symptoms (sneezing, runny nose, congestion etc.) while using Super Spray it is important to be evaluated by your regular doctor to rule out other possible causes.
Allermi formulas should help relieve nasal congestion, however you may not experience the full effects due to any structural issues blocking access to the deeper parts of your nasal passages. If you still are experiencing congestion despite using Allermi consistently for 2-3 weeks, it would be advisable to consult your ENT and consider additional interventions.
Usually both nostrils are affected by rhinitis, but sometimes one much more so than the other, so we still recommend treatment to both nostrils. However, if it is very well established that your symptoms are strictly limited to one single nostril alone (i.e., just left or just right), and particularly if that side responds only partially or not at all to your Super Spray after a few weeks of use, we strongly recommend an ENT evaluation to rule out structural anomaly or any other condition that needs medical attention.
There are several additional things you can try to help with your symptoms triggered by pet dander: use an air purifier with a HEPA filter, vacuum frequently (vacuum should also utilize a HEPA filter), bathe pets at least once weekly, do not allow pets in your bedroom, and keep pets off of clean clothes.
Smoking or inhaling any irritant can cause or worsen nasal inflammation and may also lessen the efficacy of Super Spray.
Alcohol is a known non-allergic trigger for nasal allergy symptoms. Controlling inflammation with Super Spray before drinking alcohol may help with this.
If you are taking oral antihistamines regularly, you can overlap with Super Spray for 1-2 weeks as you adjust to Super Spray and taper down to avoid withdrawal symptoms by taking ½ tablet for 7-10 days, then ¼ tablet for a 7-10 days. If you find you do need the extra effects of an oral antihistamine on high allergen exposure days, we recommend fexofenadine (Allegra) or levocetirizine (Xyzal) over all the others.
Oral decongestants such as Sudafed and Nyquil contain pseudoephedrine or phenylephrine and can cause more serious side-effects like irregular heartbeat or blood pressure changes. These medications are not safe for long-term use. You should not need to take these while on Allermi.
Newcomers to Super Spray may sneeze after using it the first few times. This is because inflamed nasal passages will be more sensitive to new nasal sprays. If you sneeze within 1 minute of taking Super Spray, wait 10 minutes and take it again if needed.
Try your best to avoid blowing your nose for 10-15 minutes after using Super Spray.
